  Applicants are required to :
  

1. Application Form
Complete the hard-copy application Application Form E
2. Supporting Documents
Submit all required physical supporting documents as indicated in the application form, including but not limited to :

• Notarized Degree Equivalence
• Indexing Certificate
• Proof of payment of the examination fee
